Lithium holds a central position in the treatment of patients with bipolar disorder, but it has a narrow therapeutic range and exerts toxic effects on various organs and systems. Its effects on cardiac function are variable and may even be severe enough to necessitate admission to an intensive care unit (ICU). We present the case of a patient who developed complete atrioventricular block due to lithium intoxication. This is a rare adverse effect of this drug, and patients with underlying cardiovascular disorders may be more vulnerable to cardiac adverse effects under lithium treatment. Electrocardiographic monitoring and/or cardiologic evaluation are indicated, along with the standard examinations, for patients with a history of cardiovascular disease who are prescribed lithium.
